Below is a list of crime investigations and activities reported by the Forsyth County Sheriff’s Office in Clemmons and Lewisville last week.

• Aug. 4 — Police service, welfare check — 1400 block of Downington Road, Clemmons. Elderly male walked away from home.

• Aug. 4 — Missing person — 6000 block of Stadium Drive, Clemmons. Subject walked away from home.

• Aug. 4 — Violation of auto law, all other — 100 block of westbound I-40/Lewisville-Clemmons Road, Clemmons. Male drove with revoked license knowing same was in state of renovation.

• Aug. 4 — Police service, welfare check — 6000 block of Stadium Drive, Clemmons. Special-needs person walked away from house.

• Aug. 5 — FCSO police service, fraud — 1600 block of Trinity Garden Circle, Clemmons. Male claims someone opened a Verizon account using his information.

• Aug. 5 — FCSO disturbance/non-criminal — 2400 block of Lewisville-Clemmons Road, Clemmons. Disturbance over ice cream machine.

• Aug. 5 — FCSO police service, information provided — 1400 block of Lewisville-Clemmons Road, Clemmons. By reporting that his girlfriend hit him with his truck.

• Aug. 5 — Shoplifting — 1400 block of River Ridge Drive, Clemmons. Two males walked out of store with beer.

• Aug. 5 — Vandalism — 100 block of southbound U.S. Hwy. 421/Lewisville-Clemmons Road, Lewisville. Male threw wood at passing vehicles on the highway.

• Aug. 5 — Larceny, all other — 200 block of Lewisville-Clemmons Road, Lewisville. Unknown individual took item from construction site.

• Aug. 6 — Communicating threats, intimidation, non-physical — 100 block of Willow Trace Circle, Clemmons. Juvenile threatened adult.

• Aug. 6 — Simple assault, non-aggravated assault — 3600 block of Clemmons Road, Clemmons. Subject stated she was hit with shopping cart.

• Aug. 6 — Violation city/county ordinance-animal related — 100 block of Almont Forest Drive, Clemmons. Dog bit neighbor while walking her dog.

• Aug. 7 — driving while impaired — 2700 block of Lewisville-Clemmons Road, Clemmons. Driver rear-ended a vehicle while driving impaired.

• Aug. 7 — Overdose — 9000 block of Lasater Road, Clemmons. Subject was found passed out in vehicle.

• Aug. 8 — Dog bites and attacks — 2600 block of Neudorf Road, Clemmons. Cat was at vet’s office and bit tech while getting ears cleaned.

• Aug. 8 — Stalking — 6600 block of Gentry Circle, Clemmons. Neighbors cannot get along.

• Aug. 8 — Communicating threats-intimidation, non-physical — 6000 block of Meadowbrook Mall Court, Clemmons. Employee accused another employee of using his shampoo and foil.

• Aug. 8 — FCSO domestic disturbance — 2100 block of Hauser Road, Lewisville. Subject stated her and her daughter had an argument, daughter left residence.

• Aug. 9 — Police service, non-criminal call for service — 6200 block of Ramada Drive, Clemmons. Father reported adult son hasn’t called him.

• Aug. 9 — Police service, non-criminal call for service — 4200 block of Clemmons Road, Clemmons. Sick raccoon in park.

• Aug. 9 — Police service, attempt suicide — 6100 block of Bobbybrook Drive, Clemmons. Male cut his arm with knife.

• Aug. 10 — FCSO police service, information provided — 6100 block of Chamberlain Place, Clemmons. Subject called requesting domestic standby assistance.

• Aug. 10 — FCSO fleet vehicle accident — 1100 block of S. Peace Haven Road, Clemmons. Cruiser hit concrete light pole.

• Aug. 10 — Trouble with neighbor — 6600 block of Gentry Circle, Clemmons. Neighbors arguing about making too much noise.

• Aug. 10 — Police service, non-criminal call for service — 6400 block of Gentry Circle, Clemmons. Items missing from Amazon package.

• Aug. 10 — Shoplifting — 1400 block of River Ridge Drive, Clemmons. Subject stole three cases of beer.

• Aug. 10 — Larceny, all other — 100 block of Applewood Drive, Lewisville. Subject attempted to steal victim’s trailer.

• Aug. 11 — Vandalism — 6800 block of Hanesbrook Circle, Clemmons. Scooter vandalized in parking lot.

• Aug. 11 — FCSO police service, fraud — 6300 block of Gough Court, Clemmons. Caller reported fraud attempt.
