Below is a list of crime investigations and activities reported by the Forsyth County Sheriff’s Department in Clemmons and Lewisville last week.

• Sept. 21 — Lost/stolen license plate, 4300 block of Riverside Dr., Clemmons. Stolen or lost tag.

• Sept. 21 — Rabies exposure, 2600 block of Lewisville-Clemmons Road, Clemmons. Sick raccoon.

• Sept. 21 — Simple assault-non aggravated assault, 3600 block of Clemmons Road/Lewisville-Clemmons Road, Clemmons. Combative patient assaulted EMT.

• Sept. 21 — Aggravated assault, 4000 block of Clemmons Road, Clemmons. Female assaulted by male with blunt object.

• Sept. 21 — Hit and run accident, 2500 block of Lewisville-Clemmons Road, Clemmons.

• Sept. 22 — FCSO-police service-welfare check, 1700 block of Lakefield Dr., Clemmons. Call of check welfare.

• Sept. 22 — Dog bites and attacks, 5300 block of Moss Creek Ln., Clemmons. Dog bit owner on his property.

• Sept. 22 — Traffic accident, 2300 block of Lewisville-Clemmons Road, Clemmons.

• Sept. 22 — Breaking and entering with force, 5800 block of Styers Ferry Road, Clemmons. Subject’s door was forced in.

• Sept. 22 — Shoplifting, 1400 block of River Ridge Dr., Clemmons. Suspect stolen cigarettes from the store.

• Sept. 22 — Involuntary commitment, non-criminal detainment, 6600 block of Gentry Cr., Clemmons. Subject taken to hospital for commitment.

• Sept. 22 — Involuntary commitment, non-criminal detainment, 1800 block of Curraghmore Road, Clemmons. Female tried to harm herself.

• Sept. 22 — FCSO: all other offense — animal-related, 7100 block of Shallowford Road, Lewisville. Found dog.

• Sept. 22 — Shoplifting, 6800 block of Shallowford Road, Lewisville. Suspect stole cigarettes from the store.

• Sept. 23 — Drug violations, paraphernalia-using/equipment, 100 block of eastbound I-40/Kinnamon Road, Clemmons. Drugs located during traffic stop.
• Sept. 23 — Automobile theft, 500 block of Hauser Road, Lewisville. Unknown subject stole vehicle out of the driveway.

• Sept. 24 — Traffic accident, 4100 block of Clemmons Road, Clemmons.

• Sept. 24 — Traffic accident, 2000 block of Lewisville-Clemmons Road, Clemmons.

• Sept. 24 — FCSO police service-fraud, 200 block of Lake Dale Ct., Clemmons. Subject had unknown transactions on her debit card.

• Sept. 24 — Publishing obscene material, 5200 block of Bridge Pointe Dr., Clemmons. Unknown subject posted video to Snapchat of underage female exposing her breasts.

• Sept. 24 — FCSO DWI-Provisional, transporting tax-paid liquor, violation of auto law-all other, 4300 block of Woodbourne Dr., Clemmons. Juvenile drove a vehicle while impaired.

• Sept. 24 — FCSO police service-fraud, 800 block of Big Tree Dr., Lewisville. Fraud phone call.

• Sept. 24 — FCSO police service-information provided, 5000 block of Styers Ferry Road, Lewisville. Sweepstakes business notified of their violations of the current governor’s orders.

• Sept. 25 — Traffic accident, 2400 block of Lewisville-Clemmons Road, Clemmons.

• Sept. 25 — Shoplifting, 2600 block of Lewisville-Clemmons Road, Clemmons. Suspect stole item out of store.

• Sept. 25 — Larceny — all other, 400 block of Williams Road, Lewisville. Unknown individual took dropped phone/wallet.

• Sept. 26 — Drug violations, 2400 block of Lewisville-Clemmons Road, Clemmons. Drugs located on traffic stop.

• Sept. 26 — FCSO police service-security check, 100 block of Beechwood Dr., Lewisville. Front door located standing open after alarm.

• Sept. 26 — Driving while impaired, 200 block of Lewisville-Clemmons Road, Lewisville. Suspect operated motor vehicle while under the influence of alcohol.

• Sept. 27 — Death investigation, 3200 block of Harper Road, Clemmons. Subject found deceased in residence.

• Sept. 27 — Violation of city/county ordinance, 6200 block of Highland Brook Dr., Clemmons. Offender allowed their dog to bark.

• Sept. 27 — Police service-assist agency, 2300 block of Lewisville-Clemmons Road, Clemmons. Assisted SHP with wreck/DWI.

• Sept. 27 — Drug violations, paraphernalia — possessing/concealing equipment, 700 block of Manarda Cr., Lewisville. Drugs and paraphernalia located on traffic stop.

• Sept. 28 — FCSO — All other offense — animal-related, 2700 block of Knob Hill Dr., Clemmons. Injured cat.
